About

Dr. Cerullo graduated with honors from Georgetown University in Washington, DC, with a degree in art history and premedical studies. She went on to obtain her doctorate in medicine from Rush Medical College in Chicago, where she was inducted into the Alpha Omega Alpha medical honor society for academic achievement. During her medical school training she received numerous honors including the Janet M. Glasgow award for a female student graduating first in her class.

Dr. Cerullo completed a medical internship at Rush University and then completed her dermatology residency at the University of California, Los Angeles, where she was chosen to serve as chief resident. Throughout her years of dermatology training, she had the opportunity to work with leaders in the field, including a mentorship award to study at the University of Bologna in Italy. She has presented at numerous dermatology meetings and published in peerreviewed journals. During residency, she was also active in teaching medical students and volunteering as medical staff at Camp Wonder, a summer camp for children with severe skin disease.

After graduating from residency, Dr. Cerullo moved to Indiana and worked for several years in private practice. She has taught medical students at the University of Notre Dame medical school and has mentored residents from local family practice residencies. Dr. Cerullo is a fellow of the American Academy of Dermatology and a member of the Indiana Academy of Dermatology and the Womens’ Dermatologic Society. Her clinical interests include a wide range of pediatric and adult conditions including acne, eczema, psoriasis, and skin cancer prevention and treatment.

Introducing direct care

Direct care means we do not contract with any government or private insurance companies thereby freeing us from the increasing number of mandates and restrictions encountered in the traditional healthcare setting. This allows us the unique opportunity to spend more time with our patients all while providing affordable, transparent and high-quality care regardless of insurance type or lack thereof. We are not a concierge practice meaning there is no yearly membership fee. Under the traditional insurance accepting medical model, practices encounter numerous cumbersome requirements. The need for expensive electronic medical records and multiple staff to keep up with time-consuming paperwork related to insurance regulations, billing issues, and mandated reporting measures are just a few. This translates to higher overhead costs, increasing number of appointments per day, longer wait times, and a decline in the quality of care.

Pricing

Medical assistant visit only

Fee - $50

  • This is for injections for items such as acne cysts, psoriasis, poison ivy, allergic reactions, etc.

Time - Up to 10 minute

Fee - $75

  • One mole/one lesion check.
  • Follow up on existing patients on prior issue.
  • Follow up for refills on stable problem such as acne, psoriasis, eczema, and rosacea.
  • Follow up for warts or actinic keratosis (precancerous spots)
  • Fee includes freezing/destruction of one wart or lesion.

Time - 11 to 20 min

Fee - $100

  • One new rash
  • New acne consultation
  • Waist up skin examination
  • Fee includes freezing 2-5 warts or lesions

Time - 21 to 30 min

Fee - $150

  • Full body skin examination
  • Hair loss consultation
  • Cosmetic consultation
  • Accutane follow up
  • Limited to up to five problems/issues
  • Fee includes freezing/destruction of 2-5 warts/lesions.

Time - 31 to 40 min

Fee - $250

  • Complex skin problem(s), for example: high risk skin cancer, autoimmune skin diseases.
  • More than 5 skin problems/issues
  • Includes freezing up to 15 warts/lesions

Surgery/Procedure

  • $125: Skin biopsy
  • $200: Small excisions, destruction of one skin cancer
  • $250: Cyst excision (one), laceration repair
  • $350: Larger skin cancer excision based on size and location
  • $500: Larger skin cancer excision based on size and location
  • $800: Larger skin cancer excision based on size and location

Note: All procedures require an office visit consultation to determine the right amount of time and cost to the patient. Prices will be quoted prior to surgery.

Other fees

  • $75: Pathology cash pay fee (we will submit pathology through your insurance at your request)
  • $20: FMLA and other forms
  • $5: Fee to code your office visit if you wish to submit your visit to your insurance.
  • $20: Prior authorizations for medications, per your insurance company.
